  • Adding a diagnosis that is not already on the claim - Aiva suggests it, never applies it
  • Adding or changing a fee modifier, which is a billing decision and never automatic
  • Every other denial reason - the claim is held with the reason named, never transmitted
  • Anything that fails a hard check: a Quebec patient, incomplete billing identity, or a missing health number
  • Turning any of this on. Every switch starts off.
