Each number is coloured by who acts. “Over to” marks where someone else takes over.

Create your account
Sign up and fill in your name and practice name. Those two are all you need today - your PRAC ID, business arrangement number and facility can wait for Settings.

Sign the Information Manager Agreement
You sign it right in the app. No printing, no downloads, nothing to send anywhere. Signing is what unlocks claim creation.

Fill in your Business Arrangement
Download form AHC11236 from the same card, open it in Adobe Acrobat Reader (it will not open in a browser or on a phone), sign your half, and email it to support@aivamd.ca. Leave the Accredited Submitter section blank.

AivaMDWe counter-sign it
We counter-sign the form and email it back to you.

YouSend it to Alberta Health
Email the fully signed form to Alberta Health with a void cheque. It tells them AivaMD sends claims for you.

Upload the completed form
Drop the fully signed AHC11236 onto the Business Arrangement card in Settings. PDF, PNG or JPG.

AivaMDWe verify your account
Your dashboard unlocks and your free trial starts: 14 days, or 30 if a colleague referred you. Every feature is open during the trial, including the Aiva assistant.

This runs alongside everything above and takes the longest. Wait for Alberta Health to confirm it is active before you send real claims - you can look around and save drafts in the meantime.

What can I do before I am verified?
You can open Settings: edit your profile, sign or re-sign the agreement, upload or replace your Business Arrangement, copy your referral link and download your data. Claims, Aiva and Connect Care open once we verify you.
When does my free trial actually start?
On the day we approve your account, not the day you sign up.
What happens when the trial ends?
You can still open and read every claim and patient you made. To keep creating and sending claims, pick Pay Per Claim, Pro or Clinic on the Billing page. The Aiva assistant comes with Pro and Clinic.
Can I send real claims as soon as I am unlocked?
Not until Alberta Health confirms your Business Arrangement is active. Until then, look around and save drafts.
I do not have Adobe Reader.
Form AHC11236 only opens in Adobe Acrobat Reader, which is free. It will not open in a web browser or on a phone.
